Create a Register Form

  1. Use the Search Bar in the top banner and type Forms.
  2. Click Create and select Register from the options.
  3. Enter the Form details
OptionDescription
* NameEnter the name of the form. This will only show to customers if Show Form Info is turned on. Multilanguage Available.
DescriptionAdd an optional description. This will only show to customers if Show Form Info is turned on. Multilanguage Available.
Show Company Info ToggleTurn this on to display the company’s logo and name at the top of the register form.
Show Form Info ToggleEnable to show the form’s name and description at the top of the form. If Show Company Info is also enabled, the form info appears underneath it.
Include Red Validation Text ToggleEnable this toggle to display red validation text under required fields for clearer guidance.
Text ColorChoose the text color for the form’s submit button.
Button ColorChoose the background color for the form’s submit button.

  1. Click Save

Register Form Customer Required Fields Tab

The Customer Required Fields Tab in a Register Form’s settings enables you to specify the information contacts must provide when completing the form. This feature is especially useful for collecting essential details from potential new customers or updating profiles of existing ones.

Key Features:

  • New Customer Profiles: If the contact is new, the information entered will create their customer profile automatically.
  • Existing Customers: For returning contacts, the details provided will update their existing profiles.

First NameSelect this field if you want the contact to enter their first name.
Last NameSelect this field if you want the contact to enter their last name.
Email AddressSelect this field if you want the contact to enter their first email address.
Phone NumberSelect this field if you want the contact to enter their phone number.
Date of BirthSelect this field if you want the contact to enter their date of birth.
GenderSelect this field if you want the contact to enter their gender.
BranchSelect this field if you have multiple branches and want the contact to select their preferred branch.
ReferenceSelect this field if you want the contact to enter their reference method.
MarketingSelect this to provide an option for the contact to give consent to receive marketing communications.

Register Form Sections Tab

The Sections Tab in a Register Form’s settings allows you to expand your form with custom options.

Add a New Register Form Section

  1. Use the Search Bar in the top banner and type Forms.
  2. Select the register form you want to modify.
  3. Go to the Sections Tab and click Add Section.
  4. Enter the Section Configurations
OptionDescription
* NameEnter the name of the section. This will be visible to both staff and customers. Multilanguage Available.
DescriptionOptionally add a description. This will be displayed at the top of the section to provide context or instructions for filling out the fields. Multilanguage Available.

* Denotes required input.

  1. Click Save.

Add Items in the Register Form Additional Fields Tab

If you want to gather additional data beside the customer field you are able to add additional items to show below. You can include Dynamic Tags or Elements:

  • Dynamic Tags: Add predefined options that appear as checkboxes in the form. If the dynamic tag includes a secondary value, such as a text field or rating scale, this will also be displayed based on the tag’s configuration.
  • Elements: Include more flexible input types, such as text, HTML, signature fields, input boxes, date fields, or document uploads.
  1. Use the Search Bar in the top banner and type Forms.
  2. Select the register form you want to modify.
  3. Go to the Additional Fields Tab.
  4. Click Add Item.
  5. Choose from the following options:

Dynamic TagsClick on dynamic tags to make them checkbox options. For example, if this form can be found in your website’s skincare page, tags like “Acne,” “Fine Lines,” or “Hyperpigmentation” can be added as choices. Your customer can select those that apply. This will help you target their marketing communication. 
Text ElementAdd simple text for instructions or context within the section. Multilanguage Available.
Input ElementThis adds an input box.
HTML ElementSimilar to the text element but with advanced formatting capabilities. Multilanguage Available.
Signature ElementAdd a signature field for capturing digital signatures.
Date ElementInsert a date field for selecting or entering dates.
Document ElementAdd an upload widget for customers to attach files or documents.
  1. Once you’ve selected the desired items, they are automatically added.
  2. Exit the pop-up and refine each item’s details as needed. For example:
  1. Add content for Text and HTML elements.
  2. Configure required inputs or formatting options.
  3. Drag and drop items to rearrange them in your desired order.

This is an example of a register form with only Email as the customer required field and show company info, show form info, and red validation text all off.

Register Form Dynamic Tag Tab

The Dynamic Tag Tab in a Register Form’s settings allows you to select dynamic tags you would like to add to the customer once they fill out this form.

Register Form Customers Tab

The Customers Tab in an Intake Form’s settings allows you to view all of the customers who have completed this form.

Register Form Integrations Tab

The Integrations Tab in a Register Form’s settings provides tools to seamlessly share or embed your form across multiple platforms, ensuring maximum reach and ease of use.

Key Features of the Integrations Tab

  1. Form Link:
    • Copy a direct link to your register form.
    • Share it via chat, include it in digital marketing campaigns, or in your social media links.
  2. QR Code:
    • Download a QR code linked to your form.
    • Use it in physical promotions like flyers, posters, or in-store displays for easy customer access.
  1. HTML Code:
    • Generate embeddable HTML code for your form.
    • Integrate it directly into your website or landing pages for a seamless user experience.
